Portland is an unusual academic market because so much of the gravity sits on one hill. Oregon Health and Science University occupies Marquam Hill above downtown, where OHSU Hospital, Doernbecher Children's Hospital, the Knight Cancer Institute, the OHSU School of Medicine, and the VA Portland Health Care System sit together, and Providence Health and Services, Legacy Health, and Kaiser Permanente Northwest carry much of the rest of the metro's clinical load.

Cost of living for a physician household

Portland sits in the middle of the western housing map. An attending salary covers more here than in Seattle or the California coastal metros and less than in the Mountain West, and inventory close to Marquam Hill is tight because the hill is hard to build on.

The tax structure is the thing to understand before you set a budget, because it is genuinely different from most states. Oregon has no statewide sales tax, which changes the arithmetic on everything from furnishing a house to buying a car. In exchange, Oregon leans on income tax, and the Portland area layers two local personal income taxes on top of it, the Metro supportive housing services tax across greater Portland and the Multnomah County Preschool for All tax. Both apply above income thresholds, which means an attending household feels them and a resident household often does not.

That combination is why so many physician families look at Clark County, Washington, across the Columbia River, where there is no state personal income tax but there is a sales tax. There is no universally right answer, only a household specific one. Where Portland physicians work

Marquam Hill is the academic center. OHSU Hospital is a teaching hospital and Level I trauma center, Doernbecher Children's Hospital sits adjacent to it, and the Knight Cancer Institute, the OHSU School of Medicine, and the VA Portland Health Care System are on the same hill. Because parking up there is limited, OHSU expanded into the South Waterfront district below, and the two campuses are connected by the Portland Aerial Tram, which is a commuter link rather than a novelty.

Providence Health and Services runs hospitals across the metro, including Providence St. Vincent Medical Center on the west side, Providence Portland Medical Center on the east side, and Providence Milwaukie Hospital to the south. Legacy Health operates Legacy Emanuel Medical Center, which houses Randall Children's Hospital, along with Legacy Good Samaritan Medical Center and Legacy Mount Hood Medical Center in Gresham.

Kaiser Permanente Northwest is a major employer here in a way it is not in most cities, running Sunnyside Medical Center in Clackamas and Westside Medical Center in Hillsboro, and Adventist Health Portland serves the southeast side. Map your actual campus first, because a Hillsboro job and a Marquam Hill job point a household at opposite ends of the metro.

Weather and seasons

Be honest with yourself about the gray. Portland runs a long wet season from roughly October into spring, with steady overcast and rain rather than dramatic storms, and it is the single thing physicians moving from the Southwest or Southeast underestimate. Households that thrive here usually treat it as a hiking and skiing season rather than something to wait out.

The reward is summer. From July into September the region is dry, warm, and long on daylight. Snow in the city is uncommon but an ice storm can shut down a hilly commute for a day, and air conditioning is worth confirming rather than assuming in older housing stock.

Recreation and family life

Recreation is a real recruiting factor in Portland rather than a nice sentence in a job posting, and it polled third nationally at 12%. Forest Park sits on the west side with miles of trail inside the city limits, Washington Park holds the Oregon Zoo, the International Rose Test Garden, and the Portland Japanese Garden, and the Willamette River carries the paddling and rowing.

Outside town, Mount Hood National Forest begins about an hour east and gives the region hiking and lift served skiing, and the Columbia River Gorge National Scenic Area runs east along the river with waterfalls and trailheads close enough for an afternoon. The Oregon Coast is a drive west, and it is a cold water rugged coast rather than a swimming beach, which is worth setting expectations on for a family coming from Florida or Southern California. Schools and education

The metro is a collection of separate districts, so the school question is an address question. Portland Public Schools covers much of the city. The Beaverton School District serves the west side, the Lake Oswego School District and the West Linn-Wilsonville School District cover the south, Tigard-Tualatin School District sits in between, and David Douglas, Centennial, Parkrose, and Reynolds serve areas on the east side.

Verify the assigned school for a specific address with the district itself rather than trusting a listing, because boundaries do not follow city lines cleanly here. Independent schools have their own admissions calendars, so start early if that is your path. For a spouse finishing a degree or teaching, Portland State University sits downtown, the University of Portland is on the north side, and Reed College is in southeast Portland, along with OHSU itself for health sciences.

Commutes and getting away

Marquam Hill is the commute problem worth solving before you buy. Parking is limited, the approach roads are narrow, and many people who work up there use the Portland Aerial Tram from South Waterfront, the OHSU shuttles, or transit rather than driving to the top. TriMet runs MAX light rail across the metro with the streetcar serving the central city, and Interstate 5, Interstate 84, Interstate 205, and U.S. 26 carry the driving traffic. Bridge choice is a daily reality on an east side to west side commute.

Portland International Airport sits northeast of downtown and is reachable by MAX, which is a genuine convenience at five in the morning. It carries wide domestic nonstop service plus international routes including Amsterdam, Tokyo, and Reykjavik, so conferences, locums work, and flights home to family are manageable without connecting through Seattle.

We surveyed more than 1,000 physician families about what matters most when choosing a new city, and cost of living came first by a wide margin, 27%, ahead of weather (14%) and recreation (12%). In Portland the cost question is usually a tax question first, because the state has no sales tax, the income tax picture is layered, and the Washington side of the river changes it again.

Financing a Portland home as a physician

The state line matters here the way it does in any border metro. A lender has to be licensed in the state where the property sits, so the lenders available for a house in Vancouver, Washington are not automatically the lenders available in Portland. Ask about property type as well, because condominiums are common close in and in South Waterfront, and lenders review the association budget, reserves, owner occupancy, and any pending litigation before approving a unit.
