Cost of living for a physician household

Washington has no personal income tax, and for a new attending or a two physician household that is a real difference in take home pay compared with California, Oregon, or New York. The state leans on sales and property taxes instead, and it levies an excise tax on large long term capital gains, with real estate excluded.

Housing is where the money goes back. Two decades of technology hiring pushed King County housing costs well past what a single professional income used to cover, and the pressure did not stop at the city line. It moved across Lake Washington into Bellevue, Kirkland, and Redmond, north into Snohomish County, and south along I-5. An attending salary stretches further here than in the Bay Area, and much less far than in the Midwest or the Southeast.

Where Seattle physicians work

Harborview Medical Center is the anchor. King County owns it, UW Medicine operates it, and it is the designated Level I adult and pediatric trauma and burn center for Washington and the regional referral center for Alaska, Montana, and Idaho. In trauma, burns, neurosurgery, or emergency medicine, that catchment is usually why the job is attractive.

UW Medical Center runs two campuses, Montlake beside the university and the School of Medicine, and Northwest in north Seattle. The School of Medicine also runs the WWAMI program across Washington, Wyoming, Alaska, Montana, and Idaho, so a faculty appointment here can carry duties well outside King County. VA Puget Sound Health Care System is affiliated with the university and trains residents alongside it.

Research is the other half. Fred Hutchinson Cancer Center holds a National Cancer Institute comprehensive designation with the University of Washington and Seattle Children's through a shared consortium, so oncology, pediatrics, and laboratory research are connected here rather than separate worlds. Seattle Children's is the pediatric referral center for the region.

Outside the academic core, Providence Swedish operates hospitals including the First Hill campus, Virginia Mason Franciscan Health brings Virginia Mason and the former CHI Franciscan hospitals together under CommonSpirit Health, MultiCare runs hospitals south through Pierce County, and the Eastside has Overlake Medical Center in Bellevue and EvergreenHealth in Kirkland. Branding here changes often, so confirm who actually employs the physicians at a given hospital.

Weather and seasons

The stereotype is rain. The truer version is gray. The wet season runs from fall into late spring and delivers persistent drizzle rather than thunderstorms, and temperatures are mild in both directions by American standards. Snow is occasional, and the hills mean a storm shuts things down out of proportion to how much fell.

The daylight swing is what to plan for. Winter afternoons go dark early, and if you are working nights or long stretches of call you can go days without seeing the sun. Name that before you sign rather than discovering it in January. The trade is the summer, because the dry, warm stretch from midsummer into early fall is long. Late summer can bring regional wildfire smoke, which matters if anyone in the house has asthma.

Recreation and family life

Water and mountains are what people trade the gray for. Puget Sound is not an ocean beach, but it is a working saltwater shoreline with ferries you can drive onto and islands you can reach in an afternoon. The open Pacific coast is a drive, not a neighbor.

The mountains sit on both sides. Mount Rainier National Park is southeast, Olympic National Park is west across the Sound, and North Cascades National Park is northeast, while Snoqualmie Pass, Stevens Pass, and Crystal Mountain put lift served skiing inside a day trip. Closer in, Discovery Park, Green Lake, the Washington Park Arboretum, and the Burke-Gilman Trail carry ordinary weekends, and Pike Place Market, Seattle Center, and the Woodland Park Zoo carry the rainy ones. Ballard, Fremont, Capitol Hill, and downtown Kirkland are the pockets where dinner does not require a car.

Schools and education

This is a many districts metro, and the district line matters more than the mailing address. Seattle Public Schools covers the city. Around it sit Bellevue School District, Mercer Island School District, Lake Washington School District in Kirkland and Redmond, Northshore School District in Bothell and Woodinville, Issaquah School District, Shoreline School District, and Highline Public Schools. Enrollment and transfer deadlines do not wait for a July start date, so settle the district question before you narrow neighborhoods.

Higher education runs deep. The University of Washington is the flagship, with campuses in Bothell and Tacoma, and Seattle University, Seattle Pacific University, and Bellevue College fill in around it. A spouse finishing a degree or switching fields can do it without leaving the region.

Commutes and getting away

Geography decides the commute here in a way it does not in a flat metro. Lake Washington splits the region and two crossings carry most drivers, the I-90 bridge and the tolled SR 520 bridge. I-5 is the dependable bottleneck north to south, and I-405 is the Eastside spine. Living in Bellevue and rounding at Harborview means committing to a bridge twice a day. Drive the route at the hour you would actually drive it before you fall for a listing.

Transit is better than it was and still uneven. Sound Transit's Link light rail runs from Lynnwood through downtown to Seattle-Tacoma International Airport and across Lake Washington to Bellevue and Redmond, and Washington State Ferries bring commuters from Bainbridge Island and Kitsap County. Not every hospital sits near a station, so check your campus, not the system map.

Getting out of town is a strength. Seattle-Tacoma International Airport is the main base for Alaska Airlines and a hub for Delta, with a long international list across the Pacific and to Europe. For conferences, interviews, or flying grandparents in, that reach matters, and the light rail station at the terminal means you are not always paying to park.

What more than 1,000 physician families told us. We surveyed more than 1,000 physician families about what matters most when choosing a new city, and cost of living came first by a wide margin, 27%, ahead of weather (14%) and recreation (12%). In Seattle the first two arrive as one decision, because the housing premium and the gray winter are both the price of the water and the mountains.
