Cost of living for a physician household

Washington asks more of an attending salary than the Southeast and Midwest markets where a lot of physicians train, and less than the Bay Area or Manhattan. Housing carries most of that gap, which is why many residents and fellows rent through training and buy as attendings.

The same salary is taxed three ways depending on where you sleep. The District levies its own graduated income tax. Maryland layers a county income tax on top of the state income tax, so a Montgomery County address and a Prince George's County address are not the same outcome, while Virginia has a state income tax and no county income tax. The District does not tax the wages of nonresidents, so a physician working at a District hospital and living in Bethesda or Arlington pays Maryland or Virginia instead.

Property tax rates, transfer and recordation taxes, and closing customs differ across the three as well, so two houses at the same price can carry very different monthly costs. Where Washington physicians work

Inside the District, MedStar Washington Hospital Center on Irving Street is one of the region's largest and busiest hospitals and a teaching hospital for Georgetown University School of Medicine, and MedStar Georgetown University Hospital sits on the Georgetown campus. The George Washington University Hospital anchors Foggy Bottom, Howard University Hospital anchors the Howard campus, Children's National Hospital sits on Michigan Avenue, and Sibley Memorial Hospital is a member of Johns Hopkins Medicine. Cedar Hill Regional Medical Center GW Health opened on the St. Elizabeths East campus in Ward 8 in 2025, the District's first new full service hospital in more than two decades.

The federal layer is unusual. The NIH Clinical Center in Bethesda is built entirely around clinical research, Walter Reed National Military Medical Center sits directly across Rockville Pike from the NIH campus, and the Uniformed Services University of the Health Sciences trains physicians for the military health system there. Add the Washington DC VA Medical Center and a real share of this region's physician jobs are federal, with federal pay structures and hiring timelines.

In Maryland, Suburban Hospital in Bethesda is part of Johns Hopkins Medicine and MedStar operates hospitals across Montgomery and Prince George's counties. In Northern Virginia, Inova leads, with Inova Fairfax Hospital as its flagship, Inova L.J. Murphy Children's Hospital alongside it, and a University of Virginia School of Medicine campus hosted within it. Georgetown University School of Medicine, the George Washington University School of Medicine and Health Sciences, and the Howard University College of Medicine all sit inside the District. For a spouse outside medicine, the agencies, universities, and contractors here make this a deep professional market.

Weather and seasons

Washington gives you four real seasons with a humid summer. The heat runs long, spring and fall are the payoff, and the cherry blossoms around the Tidal Basin are a genuine event, not a tourism slogan. Winters are variable, and some years bring a snow event that stops the region, which matters when you are covering the service.

One practical item before you write an offer. Low lying parcels near the Potomac, the Anacostia, and their tributaries can sit in FEMA mapped flood zones, and flood insurance changes the payment, so ask for the determination early.

Recreation and family life

Rock Creek Park runs through the middle of the District as a National Park Service unit, with trails, a nature center, and weekend road closures. The Chesapeake and Ohio Canal towpath begins in Georgetown and follows the Potomac upriver, Great Falls Park on the Virginia side puts whitewater close to downtown, and the Mount Vernon and Capital Crescent trails carry the region's running and cycling.

The Smithsonian museums and the National Zoo are free, which quietly changes what a weekend costs with young children and a call schedule. Longer trips run west to Shenandoah and the Blue Ridge, east to the Chesapeake Bay and the Eastern Shore, and on to the Delaware and Maryland beaches. Walkability polled low nationally at 2%, but it is real close in and in Arlington, Alexandria, Bethesda, and Silver Spring.

Schools and education

This is where the jurisdiction question stops being abstract, and the spouse running the search usually hits it first. In the District, DC Public Schools assigns in boundary seats by address, and the My School DC lottery covers charter schools, citywide schools, and out of boundary seats in one application with its own calendar. If you are moving mid cycle, learn that calendar first.

In Maryland, Montgomery County Public Schools and Prince George's County Public Schools assign by address and run consortium choice and magnet programs admitting by application. In Virginia, Fairfax County Public Schools, Arlington Public Schools, Alexandria City Public Schools, and Loudoun County Public Schools each set their own boundaries and specialty programs, and boundaries move, so verify the assigned school with the district itself. For a spouse finishing a degree or teaching, Georgetown, George Washington, Howard, the University of Maryland in College Park, and George Mason are inside the commute shed.

Commutes and getting away

Washington Metrorail is the spine, and for a physician at a campus near a station it can be the entire commute. The Silver Line now runs to Washington Dulles International Airport and into Loudoun County, which changed what a Northern Virginia address means. Where rail does not reach, the Capital Beltway, Interstate 395, Interstate 66, and Interstate 270 do.

Drive it yourself, at the hours you will actually work. Crossing the Potomac at rush hour is a different trip than staying on one side, and a household with one partner at Inova Fairfax and the other at the Washington Hospital Center is choosing a bridge as much as a house.

Getting away is easy here. Ronald Reagan Washington National Airport is close in and mostly domestic, Washington Dulles International is the region's long haul gateway, and Baltimore/Washington International Thurgood Marshall covers the north side. Amtrak out of Union Station puts Philadelphia, New York, and the Northeast Corridor in reach without flying. That is three airports and a train station rather than one option.

What more than 1,000 physician families told us. We surveyed more than 1,000 physician families about what matters most when choosing a new city, and cost of living came first by a wide margin, 27%, ahead of weather (14%) and recreation (12%). In Washington the cost question is the jurisdiction question, because the District, Maryland, and Virginia tax the same salary differently and carry different property and closing costs.

Financing a Washington home as a physician

Physician mortgage programs are available in all three jurisdictions here. Lenders serving the region commonly offer low or no down payment options for eligible physicians and dentists, often without mortgage insurance, and many underwrite student loan debt on an income driven plan differently than a conventional loan. Eligible degrees, specialty lists, and loan limits are set by each lender, so treat each feature as that lender's program.

The Washington specific point is licensing. A lender has to be licensed where the property sits, so a loan officer who can close in Bethesda is not automatically able to close in Arlington or in the District. If you have not settled the jurisdiction question, say so up front and ask which of the three the lender covers.

Prices close in also push many physician files into larger loan amounts, and maximum loan size varies by lender. Ask about contract based closing too, because some physician loan programs allow a borrower to close on a signed employment contract before the first paycheck. Yes. A lender has to be licensed where the property is located, so a loan officer who can close a purchase in Montgomery County, Maryland is not automatically able to close one in Arlington, Virginia or in the District.
