How physician families ranked their priorities

Almost every physician move to Houston is organized around one place. The Texas Medical Center sits just south of downtown, and the institutions on and around that campus, among them MD Anderson Cancer Center, Houston Methodist, Memorial Hermann, Texas Children's Hospital, Baylor St. Luke's Medical Center, and the Michael E. DeBakey VA Medical Center, set the rhythm of a physician household's calendar long before anyone tours a house.
That matters because the person doing the touring is usually not the physician. Match Day lands, a fellowship offer arrives, or a first attending contract gets signed, and the spouse starts running the relocation while the doctor finishes the year somewhere else. Everything below is written for the two of you, and the order follows what physicians and their families told us they weigh when they compare cities.
WHAT PHYSICIAN FAMILIES RANK #1 · 27%
Cost of living for a physician household
Houston is one of the metros where an attending salary does not get immediately absorbed by housing. Texas collects no state income tax, and housing across the Houston area costs less than in the coastal cities competing for the same specialists. For a family carrying training-era debt on one income, that combination is the main reason Houston keeps landing on shortlists.
Plan for the other side of the Texas trade. The state funds schools and local government largely through property tax, so the escrow portion of a Houston payment carries more weight than buyers arriving from other states expect, and windstorm or flood coverage can add to it depending on where you buy. Texas does offer a residence homestead exemption that reduces the taxable value of a primary home, and that is a question to raise with your lender and the county appraisal district early, not at the closing table.
Rather than anchor on a number that ages badly, model your own situation with the physician mortgage calculator, and enter taxes and insurance rather than principal and interest alone. In Houston that one habit changes which price range is realistic.
Where Houston physicians work
The Texas Medical Center is a dense campus of member institutions immediately south of downtown, and it is the reason Houston behaves differently from other physician markets. MD Anderson Cancer Center, Houston Methodist Hospital, Memorial Hermann, Texas Children's Hospital, and Baylor St. Luke's Medical Center all operate there, alongside the Michael E. DeBakey VA Medical Center.
Two medical schools anchor the training pipeline. Baylor College of Medicine and McGovern Medical School at UTHealth Houston run residency and fellowship programs across the Medical Center hospitals, and their faculty and residents also staff Harris Health System, the Harris County public system that includes Ben Taub Hospital and Lyndon B. Johnson Hospital. If your spouse is also in medicine or research, that concentration is worth mapping before you pick a side of town.
Care is not confined to the core. Houston Methodist and Memorial Hermann run community hospitals across the suburbs, HCA Houston Healthcare operates hospitals and freestanding facilities around the region, and MD Anderson has locations outside the main campus. For a physician household the practical consequence is this: moving from fellowship to an attending job, or from one system to another, frequently does not require selling the house. That should change how long you expect a first Houston home to fit.

Weather and seasons
Summer is the season you plan around. Roughly June through September is hot and humid, air conditioning runs constantly, and Gulf hurricane season overlaps that same stretch. That makes flood history, elevation, and windstorm coverage part of the house decision rather than a footnote. Ask about the flood record of the specific property, not just the neighborhood, and get an insurance quote before you are under contract.
The rest of the year is what balances the ledger. Fall through spring is mild enough that outdoor life keeps going, there is no snow to clear before a pre-dawn drive, and a resident coming off a run of nights in February is not scraping a windshield. Families relocating from the Midwest or Northeast usually rate this trade favorably after one winter.

Recreation and family life
Hermann Park, the Houston Zoo, and the Museum District sit directly next to the Medical Center, which is a genuinely useful accident of geography when one parent is on call and the other needs somewhere to take the kids for two hours. Buffalo Bayou Park and Memorial Park give runners and cyclists real space close to the center of the city.
Galveston and the Gulf coast are an easy day trip when someone finally gets a weekend off. Houston's food scene is one of the reasons transplants stop describing themselves as transplants, with cuisines from dozens of national traditions represented across the metro. Professional sports, theater, and a deep youth sports calendar fill in the rest. Most of the metro is car dependent, though the area around the Medical Center, Rice Village, and the Museum District is walkable in a way much of Houston is not.
POLL RANK #4 · 11%
Schools and education
Greater Houston is not one school system. It is dozens of independent school districts, which means the school question and the neighborhood question are the same question here. Houston ISD covers the core of the city and operates a large magnet and specialty program network. Around it sit big suburban districts including Cypress-Fairbanks ISD, Katy ISD, Fort Bend ISD, Conroe ISD, Clear Creek ISD, and Spring Branch ISD, each with its own boundaries, calendars, and transfer policies.
With that many districts sharing borders, the only reliable answer comes from the district office for the exact address, and it is worth having that answer in hand before an option period starts running. Private and parochial options are broad across the metro. Higher education runs deep as well, with Rice University, the University of Houston, and Texas Southern University in the city, which keeps tutoring, enrichment, and summer program options plentiful for a family that is settling in for the long run.

Commutes and getting away
Houston is a driving metro, and the commute math should start at the hospital entrance and work outward. Freeway travel time swings hard by hour, so a route timed at 10 a.m. on a house-hunting Saturday tells you almost nothing about a 5:30 a.m. arrival or a post-call drive home. I-610, I-45, I-69, I-10, the Sam Houston Tollway, and the Grand Parkway shape most of the options. Physicians working in the Texas Medical Center often factor in the METRORail Red Line, which runs from downtown through Midtown, the Museum District, and Hermann Park to Medical Center stations, when they choose where to live.
Getting out of town is a Houston strength. George Bush Intercontinental Airport is a United hub with broad domestic and international service, and William P. Hobby Airport is Southwest's Houston hub and sits closer to the southern half of the metro. Two hubs on opposite sides of the city matter more than physician families expect. A United connection out of Intercontinental and a Southwest connection out of Hobby reach very different maps, and the one closer to your address is the one you will actually use for fellowship interviews and for the trips back to parents and in-laws that keep a long distance family functioning.
What more than 1,000 physician families told us. We surveyed more than 1,000 physician families about what matters most when choosing a new city, and cost of living came first by a wide margin, 27%, ahead of weather (14%) and recreation (12%). In Houston that shows up as a specific trade: no state income tax and housing that costs less than the coastal markets recruiting the same specialists, balanced against a property tax and insurance line that belongs in the budget from day one.
Financing a Houston home as a physician
Lenders serving Texas commonly offer physician mortgage programs with low or no down payment, no private mortgage insurance, and underwriting that treats student loan debt differently from a conventional loan. Some physician loan programs also allow a buyer to close on a signed employment contract before the start date, so a residency, fellowship, or first attending move can be finished before any Houston paycheck has arrived. Every one of those features is set by the individual lender and varies by degree, training stage, and program, so the comparison is worth doing carefully.
Dr. Home Finance is a research and matching service, not a lender, broker, or financial advisor. We track what lenders actually offer physicians in each state and connect households with bankers who run genuine physician programs. Start with the Texas physician mortgage guide for statewide program details, then get matched with a banker who lends in Houston when you are ready to compare specific terms side by side.
Can I buy a home in Houston before my residency or fellowship starts?
This is one of the main reasons physician households use these programs. Many allow a buyer to close on a signed employment contract before the first day of work, within a window each lender defines, and relocations into the Texas Medical Center are among the most common cases. The window and the documentation required vary by lender, so confirm both before you set a closing date.
How do property taxes and coastal insurance affect a Houston mortgage payment?
Escrow is the line to watch, and in Houston it carries two loads at once. Texas funds schools and local government largely through property tax, so the tax portion of a payment is already heavier than buyers from income tax states expect, and on the Gulf coast a windstorm or flood premium can stack on top of it for the same house. Two homes with the same asking price in different parts of the metro can therefore carry very different monthly costs. Ask your lender for an estimate that includes both lines, and ask the county appraisal district about the Texas residence homestead exemption for a primary home.
Do physician mortgage lenders in Houston require PMI?
Many physician loan programs are structured to avoid private mortgage insurance even with a small down payment, but that is a feature of each lender's program rather than a rule. Terms differ by degree, training stage, and loan amount. The banker you are matched with can confirm exactly how their program handles PMI in Texas.
Is flood insurance something to think about when buying in Houston?
Yes. The Houston area sits on the Gulf coast and hurricane season overlaps the summer, so flood zone status, elevation, and windstorm coverage belong in the conversation early. Ask about the flood history of the specific property, not just the neighborhood, and get insurance quotes before your option period ends so the full monthly cost is clear.
