Physician Mortgage Loans in San Diego, California

San Diego runs on three different clinical economies: UC San Diego Health and its school of medicine, the private systems led by Scripps Health, Sharp HealthCare, and Rady Children's, and a large military medicine presence anchored by Naval Medical Center San Diego. Which one hired you shapes the whole relocation, including whether you have an employment contract to underwrite. This guide covers cost of living, where physicians work, school districts, commutes, and how physician loan programs are structured in California.

Kansas City skyline at twilight with Union Station's illuminated arches in the foreground and downtown skyscrapers glowing against a purple and pink sunset sky.

San Diego pulls physician households in through three doors that barely overlap. One is UC San Diego Health and its school of medicine. One is the private systems, Scripps Health, Sharp HealthCare, and Rady Children's Hospital-San Diego. The third is military medicine, because Naval Medical Center San Diego runs a large slate of military residency and fellowship programs and Naval Hospital Camp Pendleton sits at the north end of the county.

Which door you came through changes almost everything, including whether you have an employment contract at all. The person actually running the relocation is usually the spouse, working the problem while the doctor finishes a year somewhere else.

Cost of living for a physician household

San Diego is expensive. It is not San Francisco, and for many households that gap is the reason they are looking here, but it is still coastal California and it will not feel cheap after a residency in the Midwest. Budget as though you were buying on the California coast, because you are.

Two state level facts drive the math. The state income tax is graduated, and physician pay lands in its upper brackets, so net pay trails the offer letter. Under Proposition 13 the assessment follows the sale, so the escrow figure on a house that last changed hands decades ago will not resemble yours. Inland and East County areas carry wildfire exposure, which has made homeowners insurance harder and costlier to place, so get quotes before you are in escrow.

For arriving doctors the shortfall is almost never the salary, it is the liquid down payment. A San Diego purchase asks for one that residents, fellows, and junior officers have had no years to accumulate, in the same month they are paying for the move itself. That is the gap physician loan programs were built around. Before you tour, model the payment on your real pay figure and your actual student loan balance in the physician mortgage calculator.

Where San Diego physicians work

UC San Diego Health is the academic anchor, with UC San Diego Medical Center in Hillcrest and, on the La Jolla campus, Jacobs Medical Center and the Sulpizio Cardiovascular Center. The UC San Diego School of Medicine trains there, and the VA San Diego Healthcare System sits adjacent to the La Jolla campus and is itself a teaching hospital.

The private systems are large and employ across the county. Scripps Health runs Scripps Memorial Hospital La Jolla, Scripps Green Hospital, and Scripps Mercy Hospital San Diego. Sharp HealthCare runs Sharp Memorial Hospital, Sharp Mary Birch Hospital for Women and Newborns, Sharp Grossmont Hospital in the eastern suburbs, and Sharp Chula Vista Medical Center in the South Bay. Rady Children's Hospital-San Diego is the region's pediatric center.

Military medicine is the part that makes San Diego different from Los Angeles or San Francisco. Naval Medical Center San Diego, known locally as Balboa, hosts military graduate medical education across many specialties, and Naval Hospital Camp Pendleton serves the north county base. If you are arriving on orders rather than a civilian contract, tell every lender early, because it changes which documents underwriting wants and whether a VA loan belongs in the comparison.

Then there is the research economy on Torrey Pines Mesa, where the Salk Institute for Biological Studies, Scripps Research, Sanford Burnham Prebys, and the La Jolla Institute for Immunology sit near UC San Diego. For a spouse in science that cluster is a genuine job market.

Weather and seasons

Weather is the thing San Diego is sold on, and it mostly delivers. The climate is dry and mild, rain concentrates in winter, and outdoor life runs year round. For a physician coming off training in a cold city, a February evening at a park is not a small change.

Two caveats matter for house hunting. The coast spends late spring and early summer under a marine layer, so beach adjacent mornings can be overcast while communities a few exits inland are warm and sunny, and hotter still by late summer. And the further inland and the closer to canyon terrain you buy, the more wildfire exposure and insurance cost become part of the monthly number.

Recreation and family life

Ocean access polled at 5% nationally, and San Diego is one of the few metros where it is a weekday habit. The beaches run from Coronado and Point Loma through Pacific Beach and La Jolla up to Del Mar, Encinitas, and Carlsbad, and Mission Bay Park is built for paddling and running.

Inland, Torrey Pines State Natural Reserve, Cuyamaca Rancho State Park, Cleveland National Forest, and Anza-Borrego Desert State Park cover hiking and camping. Balboa Park holds the museum cluster and the San Diego Zoo. Food polled at only 3%, but the cross border influence here is real.

Schools and education

San Diego County is not one school system, and the district lines are the thing to learn first. San Diego Unified School District covers much of the city but not Carlsbad, Coronado, Chula Vista, Del Mar, Encinitas, Poway, or Solana Beach, each of which has its own. Poway Unified covers a large slice of the northern city and Poway, and San Dieguito Union High School District serves Encinitas, Del Mar, Solana Beach, and parts of Carlsbad and San Diego.

In the South Bay, Sweetwater Union High School District covers Chula Vista, National City, Imperial Beach, and the southern part of the city, and Coronado Unified and Carlsbad Unified are separate again. A San Diego mailing address does not mean a San Diego Unified assignment, so confirm the school for an exact address with the district enrollment office. For a spouse continuing a degree or a career, UC San Diego, San Diego State University, the University of San Diego, and Point Loma Nazarene University are in the metro.

Commutes and getting away

San Diego is a freeway county, and the practical question is which corridor your hospital sits on. I-5 runs the coast and connects downtown, Hillcrest, La Jolla, and the north coastal cities. I-15 is the inland spine toward Mira Mesa, Rancho Bernardo, and Escondido. I-8 runs east toward La Mesa and El Cajon. A physician at Sharp Chula Vista and one at Scripps Memorial La Jolla should not be shopping the same towns.

Transit helps in specific places. The Blue Line trolley was extended north through Old Town to University City, with stations serving the VA Medical Center, the UC San Diego campus, and the UTC center, so the La Jolla medical cluster is one of the few in Southern California reachable by rail. The Coaster commuter train runs up the coast toward Oceanside.

San Diego International Airport, still called Lindbergh Field by locals, sits close to downtown, which matters when you are catching an early flight. Its domestic schedule is deep and it carries some international service, and the Cross Border Xpress pedestrian bridge connects to Tijuana International Airport for flights into Mexico that San Diego does not serve.

What more than 1,000 physician families told us. We surveyed more than 1,000 physician families about what matters most when choosing a new city, and cost of living came first by a wide margin, 27%, ahead of weather (14%) and recreation (12%). San Diego is the metro where those two line up most directly against each other, because the weather and the coastline are exactly what you are paying the housing premium for.

Financing a San Diego home as a physician

We research physician mortgage programs at Dr. Home Finance and introduce households to loan officers. We do not lend or price loans, so everything below describes the California market, not an offer.

Physician programs from lenders licensed in California reach residents, fellows, and practicing attendings, generally with a small or zero down payment, no private mortgage insurance requirement, and student loan underwriting that can use an income driven payment instead of a percentage of the balance. None of it is uniform, so ask.

Two San Diego specifics are worth raising in the first call. Conforming limits are set at the high cost area level in San Diego County, and plenty of purchases still exceed them, so ask where a lender's physician program caps out before you tour. And if you are a military physician, ask for a VA loan priced alongside the physician program, because eligibility, funding fee treatment, and housing allowance treatment can move the comparison.

Civilian physicians relocating for a July start should ask about contract based closing, since a signed employment contract can often stand in for pay stubs and let a closing land before the start date. Run the numbers in the mortgage calculator, read the California physician mortgage guide for the statewide picture, and when you want real offers, get matched with physician loan officers working the San Diego market.

Are physician mortgage loans available in San Diego?

Yes, and the answer holds from Oceanside down to Chula Vista. Physician mortgage programs from lenders licensed in California reach residents, fellows, and practicing attendings across the county, generally with a small or zero down payment and no private mortgage insurance requirement. We research those programs at Dr. Home Finance and make introductions, and we do not underwrite or price loans, so ask the lender for the terms in writing.

How does a military physician at Naval Medical Center San Diego compare a VA loan with a physician loan?

Ask lenders to price both. A VA loan and a physician mortgage program are underwritten differently, and eligibility, funding fee treatment, down payment, and how a housing allowance is counted all differ between them. Military physicians also arrive on orders rather than a civilian employment contract, so tell each lender that early, because it changes which documents underwriting will want.

Do north county and the South Bay price differently for a physician buyer?

They do, and it is worth learning before you tour. The coastal north county cities, Del Mar, Solana Beach, Encinitas, and Carlsbad, and the La Jolla and Torrey Pines side of the city sit at the top of the county range, while the South Bay around Chula Vista and National City and the East County communities toward La Mesa and El Cajon sit lower. Conforming limits are set at the high cost area level here, so ask each lender where its physician program caps out, because that answer decides which of those bands you can actually shop.

Which San Diego school district will my children attend?

It depends on the exact address, not on the city name in the mailing address. San Diego Unified School District covers much of the city but not Carlsbad, Coronado, Chula Vista, Del Mar, Encinitas, Poway, or Solana Beach, and districts such as Poway Unified, San Dieguito Union High School District, and Sweetwater Union High School District cover their own territory. Confirm the assignment for a specific address with the district enrollment office rather than a listing site.
